What is the age requirement to join Oasis?

To join Oasis Social Day Care, individuals must be over 21 and meet the eligibility criteria.

How Long Does The Enrollment Process Take?

It would take 5-7 business days to complete the enrollment process. 

What documents or information do I need to provide during enrollment?

* Relevant medical information to ensure we tailor our services to your needs.

We request personal information such as:

  • Medicaid card | Medicare card
  • Managed Long Term Care (MLTC) health insurance card
  • Identification card
  • Medication list *

Oasis Is Someplace I Want To Be But I Don't Want My Home Attendant To Lose Their Hours?

We understand your concern about your home attendant’s work hours. It’s evident that you deeply value the support they provide.

It’s important to remember that while their work is essential, your mental and physical well-being should always be the top priority.

Your home attendant’s hours can often be adjusted to accommodate your needs and ensure you receive the care and attention you deserve.

We’re here to help find a balance that works for everyone, so please don’t hesitate to discuss any concerns or adjustments you may need to prioritize your health and comfort.

Can family members or caregivers be involved in the enrollment process?

We recognize the importance of family and caregiver involvement in the well-being of our members. 

While the enrollment process primarily focuses on the prospective member, we encourage family members and caregivers to participate in this journey. They are welcome to join you during the initial tour of our center, ask questions, and provide any necessary support.

Can I visit the center before deciding to join?

Prospects often tour the facility, meet the staff, and get a sense of the environment before committing. A one-day pass is available. Schedule your visit today.

If I sign up can my spouse, relative or friend attend?

A membership is only available to you or your loved one, and each person must enroll separately. We offer visitors a one-day guest pass. However, meals and other amenities are unavailable.
