Enrolling in Oasis Adult Social Day Care is a seamless process designed with your convenience in mind. You can quickly check your eligibility via our user-friendly website portal, where all necessary information is readily available.

If you prefer a personal touch, our friendly team is just a phone call away, ready to assist and answer your questions. For those who’d like to visit us in person, our doors are open, and our staff is eager to guide you through the enrollment process step by step.

  • Manage long term care services (Home attendant)
  • Consumer Directed Personal Assistance Program (CD PAP)*

* CD PAP is program in which a relative is assigned for you or your loved one’s care. 

  •  Elderplan / Homefirst
  • Healthfirst (Complete Care and Senior Health Partners Plan)
  • Empire Blue Cross Blue Shield (Health Plus)
  • Senior Whole Health Plan*

*Case by case basis

 

FAQs

To join Oasis Social Day Care, individuals must be over 21 and meet the eligibility criteria.

We request personal information such as:

  • Medicaid card
  • Medicare card
  • Managed Long Term Care (MLTC) health insurance card
  • Identification card
  • Medication list *

* Relevant medical information to ensure we tailor our services to your needs.

Your privacy is respected throughout our enrollment process.

It would take 5-7 business days to complete the enrollment process. 

We understand your concern about your home attendant’s work hours. It’s evident that you deeply value the support they provide. It’s important to remember that while their work is essential, your mental and physical well-being should always be the top priority.

Your home attendant’s hours can often be adjusted to accommodate your needs and ensure you receive the care and attention you deserve.

We’re here to help find a balance that works for everyone, so please don’t hesitate to discuss any concerns or adjustments you may need to prioritize your health and comfort.

A membership is only available to you or your loved one, and each person must enroll separately. We offer visitors a one-day guest pass. However, meals and other amenities are unavailable.

Before committing, prospects often tour the facility, meet the staff, and get a sense of the environment. A one-day pass is available. Schedule your visit today.

We recognize the importance of family and caregiver involvement in the well-being of our members. While the enrollment process primarily focuses on the prospective member, we encourage family members and caregivers to participate in this journey. They are welcome to join you during the initial tour of our center, ask questions, and provide any necessary support.

REFERRALS

We wholeheartedly welcome referrals from outside agencies looking to provide older adults with the best care and support.
